Job summary

Accura HealthCare of Pierce is seeking an experienced MDS Coordinator (LPN) to join our team. You will assess resident needs, prepare MDS data, and collaborate with physicians, nurses and other care professionals to ensure accurate medical records and quality care.

The role is full-time with benefits and requires participation in on-call rotation. Starting pay is $35.00/hour with increases based on experience; you will work with a supportive leadership team in a facility that values

Qualifications

• Current Nebraska LPN license, or active Nurse Compact License (NCL).
• Experience with MDS assessments and CAAs.
• Knowledge of PCC (Point Click Care) preferred.
• On-call rotation and leadership participation.

Responsibilities

• Complete all MDS assessments and Care Area Summaries (CAAs).
• Collaborate with physicians, nurses and health staff to ensure accurate medical records.
• Participate in facility quality of care and Leadership Team meetings.
• Be part of On-Call nursing management team.
• Perform essential functions of the job with or without accommodation.

JOB SUMMARY:

The MDS (Minimum Data Set) Coordinator is required to be knowledgeable of applicable laws and/or licensing regulations for healthcare facilities. This includes compiling information, preparing statistical reports and data from health records, and personalizing assessments for use in the evaluation of quality of care and utilization review for the purpose of clinical reimbursement. The MDS Coordinator confers with physicians, nurses, and other health personnel to ensure complete, current, and accurate medical records.

JOB RESPONSIBILITIES:

• Current State of Nebraska Licensed Practical Nursing (LPN) license, or an active Nurse Compact License (NCL)
• Complete all Minimum Data Sets (MDS) assessments and Care Area Assessment Summaries (CAAs)
• Participate in the facility quality of care and Leadership Team meetings
• Knowledgeable with Point Click Care (PCC) is preferred
• Will be part of the On Call nursing management team
• Must be capable of performing the Essential Functions of this job, with or without reasonable accommodation.
