As a MDS/Care Plan Coordinator at Butterfield Trail Village, you will play a pivotal role in assessing and monitoring the health and wellbeing of our residents in accordance with the Resident Assessment Instrument (RAI) and relevant legislation. Your responsibilities will include performing comprehensive care area assessments and guiding the care planning process, ensuring that the health and wellbeing of our residents are consistently monitored and accurately documented. To excel in this role, you should possess prior nursing experience, a sharp eye for detail, and proficiency in data collection and analysis. Exceptional interpersonal skills, a deep understanding of patient care protocols, and expert knowledge of MDS activities are essential for success.
