Qualifications

• Must have a minimum of two years of experience in a post-acute/skilled nursing facility, LTC or hospital.

Responsibilities

• Perform assessments of new patients and transmit MDS report forms.
• Plan and coordinate educational and on-the-job training programs.
• Develop and maintain records and reports pertinent to in-service educational programs.
• Work with the business office to ensure correct benefits packages for employees.

Clear & Active LVN/LPN or RN license in the State
