Job summary

Avir Vista seeks a licensed MDS Coordinator to join our family-focused team. The ideal candidate will lead the MDS process, ensure accurate RAI completion, and maintain compliance with State and Federal regulations.

Responsibilities include coordinating with facility staff, validating transmissions to the State, and guiding a high-performing team to deliver quality resident care while upholding our family-centered culture.

Qualifications

• Valid Texas RN license.
• CPR certification required.
• Minimum 1 year supervisory experience in hospital or SNF.
• Knowledge of long-term care regulations and state/federal standards.

Responsibilities

• Complete the RAI process for each resident.
• Transmit and verify receipt by the State as required.
• Lead a high-performing MDS team and ensure regulatory compliance.
