Paradigm Healthcare's Paradigm at the Creek in Wharton, TX seeks an MDS Coordinator. Full-time day shift with a $3,200 pay and sign-on bonus. Responsibilities include MDS maintenance, ARD planning, and coordination of assessments in a nursing facility.

Qualifications include an active LVN/RN license in Texas, 2 years of long-term care experience, and RAI process familiarity. Proficiency with Point Click Care and MS Office is preferred.

Qualifications

• Must possess an active, unencumbered LVN/RN license in the state.
• Registered Nurse License preferred.
• 2 years’ Long-Term Care experience preferred.
• 6 months experience in rehabilitative/restorative nursing practice preferred.
• 2 years’ experience in RAI process completion preferred.

Responsibilities

• Maintain the current MDS status of each resident in accordance with applicable laws and facility policies.
• Schedule residents for OBRA, Medicare PPS, and Medicaid assessments and inform departments/families.
• Monitor clinical systems for MDS-related changes and coordinate reassessment.
• Set ARD with the interdisiplinary team to reflect resident status accurately.
• Complete MDS and CAAs per policies and guidelines.
• Attend daily Casemix, weekly Utilization Review, and monthly Triple Check meetings.
• Maintain assessments in the active clinical record for at least 15 months.
• Transmit MDS data to the state per regulations and facility standards.
• Adhere to facility policies for day-to-day nursing service operations.
• Participate in surveys by government agencies as needed.
• Contribute to planning and implementing the care plan to achieve wellbeing.
