Paradigm at the Creek in Wharton, TX is seeking an MDS Coordinator to manage MDS updates, ARD planning, and regulatory compliance for residents. The role involves coordinating assessments, scheduling, and collaboration with the interdisciplinary team to ensure optimal care plans.

The position offers a day shift, a $3,200 pay, and a sign-on bonus. Licensure in TX is required, with RN preferred; Long-Term Care experience is valued. Benefits include comprehensive health coverage and PTO.

Qualifications

• Must possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse or Registered Nurse in the State (TX).
• RN License preferred; LVN/LPN acceptable with facility experience.
• Minimum 2 years’ Long-Term Care experience; RAI process experience encouraged.
• Ability to make independent decisions and lead interdisciplinary teams.
• Strong knowledge of patient care, scheduling, and regulatory compliance.

Responsibilities

• Maintain the current MDS status of each resident per laws and facility policies.
• Schedule residents for required assessments and communicate with departments and families.
• Monitor MDS process and coordinate reassessment and care plan updates.
• Set ARD with the team to reflect resident needs and services.
• Complete MDS and CAAs accurately per guidelines.
• Coordinate daily Casemix, weekly UR, and monthly Triple Check meetings.
• Maintain assessments for 15 months per Federal Regulations.
• Transmit MDS data to state in compliance with regulations.
• Educate residents, families, and caregivers when needed.
• Provide CPR and emergency support per physician orders.
