Job summary

Signature HealthCARE at Colonial Rehab & Wellness Center is seeking an MDS Candidate who is detailed, organized, and bedside-capable to assess residents and interact with families. This role emphasizes accurate data collection from medical records, interviews, and assessments.

You will participate in PPS and Medicare meetings, ensuring billing compliance, and regularly review Care Plans to reflect changes. A minimum of 3 years’ clinical experience and at least 1 year in MDS is required.

Qualifications

• Licensed Practical/Vocational Nurse with current state licensure.
• Minimum three (3) years of clinical experience; long-term care preferred.
• Minimum one (1) year of MDS experience.
• Collect information to complete the MDS using records, assessments and interviews.

Responsibilities

• Collect information to complete the MDS using the medical record, bedside assessment, and staff, resident and/or family interviews.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.
• Review Plan of Care at least quarterly and with each Comprehensive Assessment to assure changes during the quarter are included and updated.
