Job summary

Desoto-Nursing- in Texas is seeking an experienced MDS Coordinator to ensure accurate assessments, regulatory compliance, and effective care planning for residents. The role requires reliability, strong organization, and collaborative cross-team coordination.

You will oversee resident assessments, maintain MDS data integrity, lead interdisciplinary meetings, update care plans, and provide quality reporting to leadership.

Qualifications

• Current Nursing License in good standing.
• Experience in geriatric nursing.
• Excellent assessment, documentation, and communication skills.
• Understanding of nursing home regulations and compliance.

Responsibilities

• Coordinate admissions, annual, quarterly, Medicare, and change-of-condition assessments with timely department communication.
• Ensure regulatory compliance with Federal and State guidelines for MDS.
• Monitor documentation and care plans for accuracy and organization.
• Utilize MDS software for correct data entry and reporting.
• Lead interdisciplinary meetings with Medicaid, ITTM, and care plan teams.
• Update and revise care plans based on current assessments.
• Provide quality reporting of indicators to leadership.
• Submit MDS data per reporting standards.
• Stay current by attending professional development sessions.
• Oversee ancillary data for reporting to corporate billing.
