M2 HEALTHCARE LTD in New York (Brooklyn) seeks an experienced PRI Assessor and RN to plan, organize, and evaluate Medicare assessment programs and act as Employee Health Nurse for the Brooklyn Gardens Nursing and Rehabilitation Center.

Responsibilities include timely Medicare determinations, coordinating MDS+ with staff, and ensuring documentation compliance across shifts. Strong knowledge of Medicare regulations and excellent communication are essential for success.

Qualifications

• Bachelor’s degree preferred and NY RN license.
• PRI Assessor certification and MDS+ training completed.
• Knowledge of Medicare regulations in RHCF.

Responsibilities

• Ensures Medicare determinations are completed in a timely manner.
• Informs Director of Nursing, Business Office and Social Service of determinations.
• Assesses documentation required for Medicare coverage.
• Completes Certification and Recertification forms and skilled care forms.
• Coordinates MDS+ with Unit Coordinator and related disciplines.
• Delegates MDS+ sections to staff with clinical knowledge.
• Establishes policies so shifts know resident information on MDS+.
• Coordinates resident assessment (MDS+) and care planning (RAPS).
• Participates in Interdisciplinary Resident Care Plan meetings.
• Coordinates quarterly PRI submissions for RUG reimbursements.
• Participates in committees influencing nursing practice.
• Coordinates Employee Health Services in the Nursing Home.
• Follows safety procedures and respects residents’ rights.
• Performs related and emergency duties as required.
