THRYVE Healthcare in Chicago, IL is seeking a skilled MDS Coordinator to join our clinical team. You will assess residents using MDS 3.0, develop care plans with an interdisciplinary team, and ensure accurate documentation in the EMR.

Qualifications

• 2 years of experience as an MDS Coordinator in long-term or acute care.
• Strong knowledge of MDS 3.0 assessment and care planning.
• Excellent communication, organization, and discretion.

Responsibilities

• Coordinate and conduct resident assessments, including the MDS 3.0 assessment.
• Develop, implement, and maintain resident care plans in collaboration with the interdisciplinary team.
• Utilize electronic medical records (EMRs) to document and track resident information and assessments.
• Participate in Care Plan Meetings and other interdisciplinary team meetings to ensure resident-centered care.
• Collaborate with Nursing, Therapy, and Social Services to develop and implement individualized care plans.
• Identify and report potential quality of care issues and opportunities for improvement to the Director of Nursing and other stakeholders.
