Are you an experienced MDS nurse interested in the next step? The MDS Coordinator provides oversight of the RAI process and conducts assessments and care plan coordination for guests. The MDS Coordinator supervises the Care Management Nurse, MDS Nurse.

Responsibilities

• Complete the MDS, CAA’s, and care plans within regulated time frames.
• Coordinate scheduling the RAI process with the interdisciplinary team.
• Assess residents through physical assessment, interview, and chart review.
• Discuss resident care needs with caregivers, including physicians, nursing, social services, therapy, dietary, and activity staff.
• Review information from hospitals, consults, and outside agencies for assessment and care planning.
• Coordinate, identify, and initiate significant change MDSs.
• Conduct PPS meetings, maintain MDS assessments per Medicare schedule, and monitor Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
• Stay current with American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure.
• AANC certification a plus; RAC-CT preferred.
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS).
• Experience as an MDS Nurse is preferred.
