Orchard View Post Acute is seeking an MDS Coordinator (LPN or RN) to join our dedicated team. The role focuses on accurate assessments and high-quality care planning for residents, working closely with nursing and therapy to support outcomes and regulatory compliance.

Responsibilities

• Monitor case mix index and identify opportunities for improvement
• Stay current on MDS guidelines and regulatory updates
