Job summary

Goldwater Care is seeking a MDS Coordinator to ensure accurate and timely Medicare/Medicaid case-mix documentation for proper reimbursement. You will conduct ongoing MDS reviews to optimize RUG categories and coordinate interdisciplinary teams across shifts to meet regulatory timeframes.

The role requires an RN or LPN license, prior long-term care experience, and strong English communication. Benefits include health, dental, and vision insurance, plus a 401(k) after six months of service.

Qualifications

• Experience in long-term care settings and MDS coordination preferred.
• Ability to read, write, speak and understand English.
• Strong attention to regulatory timelines and documentation standards.

Responsibilities

• Assesses health status and care level of new admissions.
• Ensures timely completion of MDS assessments including PPS Medicare, quarterly, annual, and significant changes.
• Communicates level of care to all disciplines.
• Coordinates interdisciplinary participation for MDS completion within regulatory time frames.
• Maintains accurate schedule and reference dates for MDS assessments.
• Oversees data entry and electronic submission of MDS assessments.
• Verifies submissions and maintains records.
