Job summary

Birkwood Village of Fort Madison is seeking an experienced MDS / Care Plan Coordinator to lead resident assessments (MDS) in accordance with current rules, regulations, and RAI guidelines, a key clinical leadership role shaping quality of care across our community. The role requires coordinating MDS/RAI processes, developing individualized care plans, and collaborating with nursing leadership.

Iowa RN license and familiarity with PCC are preferred; on-site schedule is full-time with on-call

Qualifications

• Current Iowa RN license in good standing.
• Experience with MDS/RAI, PDPM, and care plan coordination preferred.
• Familiarity with PointClickCare (PCC) or similar software is a plus.
• Strong organizational skills and ability to meet regulatory deadlines.

Responsibilities

• Coordinate MDS assessments, ARDs, and timely submission; stay current on RAI Manual changes.
• Review census changes, progress notes, and Medicare A documentation for significant changes in condition and skilled services.
• Develop and coordinate individualized care plans, ensuring timely completion and revision with every assessment or status change.
• Attend weekly Medicare meetings and monthly/quarterly QAPI meetings.
• Promote patient- and family-centered care, including scheduling Care Plan Conferences.
• Participate in on-call rotation, every 4th weekend.
