Crane’s Mill, a 5-star CCRC in West Caldwell, NJ, seeks a part-time MDS Coordinator (32 hours/week) to oversee MDS 3.0 processes and care planning for residents. Requires RN/LPN experience in skilled nursing, nursing leadership, and strong interdisciplinary teamwork.

Bachelor’s in nursing and NJ RN license with CPR; AN AAC certification preferred. Competitive benefits and salary up to $105,000 are offered.

Qualifications

• Bachelor's Degree in Nursing and RN license in NJ.
• CPR certification required.
• AN AAC certification preferred.

Responsibilities

• Coordinate all processes to complete the MDS 3.0 accurately and timely.
• Ensure MDS data supports Care Plans and resident records per regulations.
• Maintain MDS completion schedule and weekly transmissions with reports.
• Interact with residents, families, physicians, staff and regulatory agencies.
• Lead and participate in IDT meetings; document care plans and updates.
• Record notes in EMR after each Care Plan review and ensure physician certs.
• Participate in Quality Assurance activities and survey readiness.
