LPN: $38.00 - $40.00 per hour
RN: $83,000 - $87,000 annually
The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI)/MDS process to ensure accurate, timely, and compliant assessments. This role works closely with the interdisciplinary team to support quality resident care, reimbursement, and regulatory compliance.
Current, active LPN or RN license in the Commonwealth of Virginia
Minimum 1 year of MDS experience required
Experience in a skilled nursing or long-term care setting preferred
Strong knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and PDPM
Excellent assessment, documentation, and communication skills
Ability to work collaboratively with an interdisciplinary team
Complete accurate and timely MDS assessments in accordance with federal and state regulations
Coordinate the Resident Assessment Instrument (RAI) process
Ensure compliance with CMS guidelines and reimbursement requirements
Collaborate with nursing, therapy, social services, dietary, and other departments to develop comprehensive care plans
Monitor assessment schedules and ensure documentation deadlines are met
Review medical records for accuracy and completeness
Participate in quality assurance and performance improvement initiatives
Stay current with regulatory changes affecting MDS and reimbursement
