Job summary

Riverside Health and Rehabilitation Center in Trenton, NJ seeks an experienced MDS Coordinator to oversee MDS assessments, ensure regulatory compliance and appropriate reimbursement. The role integrates with the clinical team to maintain high care standards.

Qualifications

• Current and valid New Jersey RN license.
• Prior MDS Coordinator experience required.
• Strong knowledge of MDS 3.0, RAI guidelines, PDPM, Medicare and skilled nursing regulations.
• Experience with PointClickCare preferred.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes
• Develop and maintain assessment schedules for Medicare, Medicaid, and OBRA residents
• Ensure compliance with federal and New Jersey regulations
• Lead interdisciplinary care-plan meetings and ensure care plans reflect residents’ needs
• Review clinical documentation for accuracy, completeness, and reimbursement support
• Monitor Medicare eligibility, skilled coverage, and assessment requirements
• Apply PDPM guidelines and identify appropriate clinical categories
• Collaborate with nursing, therapy, social services, dietary, and other departments
• Educate clinical staff on documentation requirements and MDS-related processes
• Monitor quality measures and help identify opportunities for improvement
• Participate in audits, surveys, and utilization-review activities
