Job description

Rochester Center is hiring a MDS Coordinator in Rochester, NY!

DUTIES:

  1. Completing accurate assessments, MDS & care plans as assigned
  2. Initiating care plans and supporting activities as assigned
  3. Creating and distributing monthly care plan calendars in a timely fashion
  4. Maintaining & updating all care plans and assessments as required
  5. Monitoring & auditing clinical records, ensuring accuracy & timeliness
  6. Informing DON of persistent issues related to non-compliant documentation
  7. Protecting the confidentiality of Resident & Facility information at all times

REQUIREMENTS:

  1. MUST HAVE PRIOR MDS 3.0 EXPERIENCE
  2. Valid New York RN License
  3. Long Term Care Experience Required!
  4. Must be highly organized, professional & motivated
  5. Should have solid computer skills
  6. Excellent communication skills
  7. Should be friendly and a team worker
