Job summary

MARIES MANOR in Vienna, Missouri is seeking an MDS Coordinator to join our team. Responsibilities include accurate MDS assessments and care plans, and maintaining updated records for residents in a long-term care setting.

Qualified candidates hold an RN or LPN license, have prior MDS experience, and demonstrate strong organizational and communication skills. This is a full-time, on-site role with a benefits package including health and retirement offerings.

Qualifications

• Valid RN or LPN license is required.
• Must have prior MDS experience in long-term care.
• Experience with MDS and care plans.
• Long-term care experience required.
• Must be able to work 40 hours per week.
• Excellent communication and teamwork skills.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times
• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness

MDS Coordinator Qualifications and Skills:

• Knowledge of skilled nursing facilities survey process and regulations would be helpful.
• Must have prior MDS Experience
• Long Term Care Experience Required

• Must be highly organized, professional & motivated
• Should have solid computer skills
