Qualifications

• Valid Texas Nursing License.
• CPR certification required.
• Minimum 1 year supervisory experience in a hospital, skilled nursing facility, or related healthcare program.
• Knowledge of long-term care standards per Federal Register and State regulations.

Responsibilities

• Complete the MDS/RAI process for each resident in the facility.
• Transmit and verify receipt of MDS/RAI data with the State.
• Build and lead a high‑performing MDS team and culture.

We are searching for a Licensed MDS Coordinator who is resident & family focused, a team builder, and excited about the opportunity to build and lead a high-performing team and culture. The primary purpose is to complete the RAI process for each resident of the facility, as required byState and Federal regulations. This includes transmission and verification of receipt by the State.

Qualifications for MDS Coordinator:

• Valid Texas Nurse License.
