Job summary

Mental Health Association of Rochester seeks an MDS Coordinator (RN) to lead resident assessments and care planning on-site.

You will manage the MDS/RAI process, ensure timely, accurate assessments, and maintain compliant documentation while analyzing clinical data to support quality outcomes and reimbursement accuracy.

Qualifications

Role:

MDS Coordinator, RN

Responsibilities:

Lead resident assessment and care planning, manage the full MDS/RAI process, ensure timely and accurate assessments, maintain compliant documentation, analyze clinical data, support reimbursement accuracy, and promote quality outcomes.

Key Duties:

Collaborate with interdisciplinary team to develop individualized, resident-centered care plans, monitor and improve resident safety, comfort, and independence, and ensure compliance with regulations.

Qualifications & Skills:

Registered Nurse (RN) license, strong knowledge of MDS/RAI processes, clinical data analysis skills, attention to detail, and ability to work collaboratively in a team environment.
