We are currently looking for the ideal RN MDS Coordinator for our facility. You must have a current nursing license in IL. The primary purpose of your job is to provide direct nursing care to the residents and supervise the day-to-day nursing activities performed by the nurse aides, in accordance with current federal, state and local standards, guidelines and regulations that govern Friendship Manor, and as may be required by the Director of Nursing, to ensure that the highest degree of quality care is maintained at all times.

MAJOR DUTIES AND RESPONSIBILITIES
- Develops treatment plans appropriate for patients that comply accordingly to MDS guidelines.
- MDS coordinators are tasked in completing various MDS paper works; submit them on time and make certain it meets with the current guidelines and regulations as these regulations may vary each year.
- Coordinate the completion of the resident assessment (MDS), Triggers and RAPs in accordance with current rules, regulations and guidelines
- Ensure completion and submission of MDS data within regulatory timeframes
- Coordinate the development of comprehensive assessments and care plans of each resident
- Conduct/coordinate facility daily PPS meetings and IDT Process and Claim review
- Assist with education and overall plan of care
- Cooperate with monitoring and audit functions and investigations
- Participate in the review Quality Indicator report and Resident Specific Profile
- Participate in educational programs to maintain knowledge current with RAI/PPS regulations
- Develop and maintain a good working relationship with inter-department personnel, as well as other departments within the facility, to assure that medical records can be properly maintained
- Measure wounds when necessary
- Prepare necessary reports for monthly CQI meeting.
- Attend family conferences
- Assist administrator with preparation of weekly meetings
- Collaborate with other clinical staff and run assessment care plan meetings.
QUALIFICATIONS
- Must be a state-licensed RN
- Must have 1 - 2 years MDS experience preferably in a Long-Term Care or Rehab environment
- PCC experience preferred
- Must be knowledgeable of long-term care nursing and medical practices and procedures, as well as legal regulations and guidelines that pertain to long-term care.
- Must have excellent customer services skills and teamwork attitude.
- Excellent written and verbal communication skills
- The ability to multitask and oversee functions at the facility on a daily basis.
- Must be able to work independently and make decisions when circumstances warrant such action.
- Must maintain current CPR certification
