Role Description

The MDS Coordinator is a full-time, on-site role located in Maywood, NJ. This position is responsible for completing and overseeing the Minimum Data Set (MDS) assessments, ensuring accuracy, timeliness, and compliance with state and federal regulations. The MDS Coordinator collaborates with nursing, therapy, and interdisciplinary team members to gather clinical information and translate it into comprehensive care plans. Daily responsibilities include reviewing resident documentation, monitoring changes in condition, updating assessments, and coordinating care planning meetings. The role also involves optimizing reimbursement through accurate coding, supporting quality improvement initiatives, and providing education to staff on MDS and care planning processes.

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing, with experience in long-term care or skilled nursing settings.
• Demonstrated proficiency with MDS assessments, documentation standards, and related regulatory requirements.
• Strong skills in Care Planning and Care Plans, including interdisciplinary collaboration and resident-centered approaches.
• Experience in Geriatric Nursing, with a solid understanding of the clinical needs and challenges of older adults.
• Knowledge of reimbursement systems (Medicare/Medicaid), RAI guidelines, and facility quality measures.
• Excellent attention to detail, analytical skills, and ability to manage multiple priorities and deadlines.
• Effective written and verbal communication skills, including the ability to educate and support clinical staff.
• Familiarity with electronic health records and MDS software; basic proficiency in common office applications.
