Job summary

The Suites Pasadena is seeking an MDS Coordinator to join our team in Pasadena, TX. The role focuses on overseeing resident assessments and developing comprehensive care plans in collaboration with nursing, therapy, nutrition, and other teams to meet regulatory standards.

RN or LVN license is required and prior experience in a Skilled Nursing Facility is preferred. This is a full-time, in-person position with background check and compliance with Eduro Healthcare values.

Responsibilities

• Assess residents behavioral, psychological, clinical, nutritional, and physical strengths and needs to create an individualized care plan that considers the resident holistically.
• Coordinate with nursing, PT, nutrition, and other disciplines to ensure care plan guidelines are followed and compliant with ethical standards and Medicare requirements.
• Oversee and facilitate completion of resident assessments and planning of care while ensuring compliance with Federal and State regulations.

The MDS Coordinator will oversee and facilitate the completion of resident assessments at nursing facilities. Throughout the assessment and while planning care, they ensure compliance with Federal and State regulations.

MDS Coordinator Responsibilities:
