Job summary

Litchford Health & Rehabilitation Center in Raleigh, NC is seeking an RN MDS Coordinator to ensure accurate Medicare MDS assessments and collaborate with the interdisciplinary team to develop care plans.

You will review patient records, communicate with staff, the patient's physician, and family, and participate in care conferences to ensure ARD compliance and high-quality documentation. This is a salaried exempt position.

Qualifications

• LPN or equivalent with MDS experience preferred.
• Experience in Skilled Nursing / Long-Term Care MDS.
• Proficiency with MS Office.
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Complete and submit accurate minimum data set (MDS) assessments on patients.
• Oversee care plan process including coordination of patient care plan conferences.
• Ensure ARD stays within Medicare & Medicaid guidelines.
• Participate in monthly Quality Measure meetings.
• Work with nursing leadership to identify and resolve documentation opportunities.
• Assist interdisciplinary team in Medicare coverage decisions.

DESCRIPTION

As a RN MDS Coordinator your primary responsibility will be to ensure that every patient has an accurate minimum data set (MDS) assessment and work closely with the interdisciplinary team to create comprehensive plans of care after reviewing the patient’s medical record and communicating with direct care staff, the patient’s physician, and family. You will play a pivotal role in helping us meet our commitment to providing Care Beyond Compare.

What You’ll Do

• Complete and submit accurate minimum data set (MDS) assessments on patients.
• Oversee care plan process including coordination of patient care plan conferences.
• Ensure Assessment Reference Date (ARD) stays within Medicare & Medicaid guidelines.
• Participate in monthly Quality Measure meetings.
• Work with nursing leadership team to identify and resolve opportunities in documentation.
• Assist interdisciplinary team in making appropriate Medicare coverage decisions.

What You Need

• Licensed Practical Nurse (LPN) will be considered with relevant experience.
• Skilled Nursing / Long-Term Care MDS experience preferred.
• Proficient with computer programs including Microsoft Office Suite (Word, Excel, etc.)
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.
