Brooklyn Gardens Nursing and Rehabilitation Center is seeking a PRI Assessor who will plan, organize, implement and evaluate Medicare assessments and the MDS+ process, while serving as the Employee Health Nurse for the facility.
The role requires a NY RN license, PRI Assessor certification, MDS+ training, and at least two years of experience in a long-term care setting, with strong Medicare knowledge and computer skills to support resident care and regulatory compliance.
Bachelor’s Degree preferred. Certification as a PRI Assessor. Completion of the MDS+ Training Course. Licensed in the State of New York as a Registered Professional Nurse.
At least two years experience as a PRI Assessor in a long term care facility.

Position requires standing and sitting for long periods at a time.
Knowledge of the Medicare regulations in a Residential Health Care Facility. Excellent verbal and auditory skills required for communicating effectively and clearly with residents, families, physician and subordinates. Must have knowledge of computer.
Assumes responsibility for the planning, organizing, implantation and evaluating programs of Medicare Assessments and Minimum Data Set Plus Coordinator. Functions as the Employee Health Nurse for the Brooklyn Gardens Nursing and Rehabilitation Center.
