Moose Lake Village is a Cassia community seeking a dedicated RN to join as a Full-Time MDS Coordinator in Moose Lake, MN. You will oversee MDS and care planning, assign tasks to nursing staff, and handle RAI/MDS documentation while collaborating with the Case Manager and Admissions.

The role requires an MN RN license, RAC certification, MDS experience, and proficiency with computer software. This is a full-time position with benefits and day shifts from 8:30am to 5:00pm.

Qualifications

• RN license in Minnesota is required.
• Resident Assessment Coordinator certification is required.
• Experience with MDS is preferred.
• Proficient in computer software.

Responsibilities

• Oversee and complete MDS and care planning.
• Manage and assign tasks to nursing staff on the unit.
• Complete documentation for RAI and MDS assessments and observations.
• Develop initial and temporary care plans.
• Assist Case Manager with Medicare scheduling, monthly billing, charting, and notices of non-coverage.
• Document and maintain paperwork, referral records and reports.
• Collaborate with Admissions staff, Case Manager, and Supply Coordinator.
• Maintain courteous and respectful interactions with residents, families, and staff.
• Perform additional tasks as needed.

Job description

Moose Lake Village, a Cassia community, has a rare opportunity for an experienced Registered Nurse (RN) to join our team as a Full-Time MDS Coordinator.

In this role, you will manage the clinical care of residents by assigning different tasks to nurses within the department. Our ideal candidate has prior LTC experience, thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills.

Position Type: Full-Time, benefits eligible position

Day Shifts: 8:30am to 5:00pm

Wage Range: $40 - $48 / hour depending on experience

MDS Coordinator Responsibilities:

• Oversee and complete MDS and care planning
• Manage and assign tasks to nursing staff on the assigned unit
• Complete all documentation for RAI and MDS assessments and observations
• Develop initial and temporary care plans
• Assist Case Manager with Medicare scheduling, monthly billing, charting, and issuing notices of non-coverage
• Document and maintain paperwork, referral records, and reports
• Work in collaboration with Admissions staff, Case Manager, and Supply Coordinator
• Display a courteous attitude and respect for all residents, families, and staff
• Perform additional tasks as needed

MDS Coordinator Qualifications:

• RN license in the state of Minnesota required
• Must have Resident Assessment Coordinator certification
• Experience with MDS preferred
• Proficient in computer software
