As an MDS Coordinator, you will play a critical role in ensuring the highest quality of care for our patients. Your primary responsibility will be to collect and maintain accurate and thorough data in our Minimum Data Set (MDS) software, ensuring compliance with regulatory requirements. You will also be responsible for facilitating the MDS process, conducting concurrent and comprehensive assessments, and developing and implementing individualized care plans. Collect and maintain accurate and thorough data in our Minimum Data Set (MDS) software
Facilitate the MDS process, including conducting concurrent and comprehensive assessments
Develop and implement individualized care plans for patients
Collaborate with healthcare professionals to ensure accuracy and completeness of MDS data
Maintain confidentiality and adhere to HIPAA guidelines
Participate in quality improvement initiatives and staff development programs
Demonstrate a commitment to patient-centered care and a passion for improving the healthcare experience
Bachelor's degree in Nursing or a related field (or equivalent experience)
Strong communication and interpersonal skills
Ability to work in a fast-paced environment with multiple priorities
