Job summary

The Springs is seeking a detail-oriented RN to serve as MDS Coordinator. You will manage the RAI/MDS process, drive care plan development, and ensure compliance with federal and state guidelines.

Qualifications

• Graduate of an approved RN program.
• Active Arkansas RN license.
• Experience in MDS coordination preferred.
• Strong documentation and regulatory compliance mindset.

Responsibilities

• Complete MDS assessments for admissions, annuals, quarterlies and significant changes.
• Perform risk assessments (Braden, fall risk, incontinence) and CAAs.
• Develop and update resident care plans with clinical team.
• Attend care plan meetings and QA activities.
• Ensure accurate documentation and train staff on MDS processes.
• Maintain Medicare/HMO documentation including ABNs and certifications.
• Collaborate with therapy, nursing, and billing on PPS and scheduling.
• Assist with audit prep and survey processes.
