Job summary

Clement Manor in Wisconsin is seeking a Part Time MDS Coordinator to join our nursing team. The MDS Coordinator oversees and coordinates resident assessments (MDS) in accordance with Federal and State rules, including PDPM guidelines, to ensure accurate documentation and compliance.

The role supports care planning and collaborates with nursing, therapy, and administrative staff. On-call rotation participation is required, with five-week scheduling blocks and holiday rotation.

Qualifications

• Graduate of an accredited School of Nursing.
• Wisconsin state licensure is required.
• AANAC certification preferred but not required.
• Understanding of Federal and State rules and regulations (PDPM).

Responsibilities

• Oversee and coordinate the development and completion of the resident Assessment (MDS) in accordance with Federal and State rules and guidelines.
• Support care planning and collaborate with nursing, therapy, and administrative staff.
• Participate in on-call rotation as scheduled (every 5 weeks) and during holiday rotation.

We are currently looking for a Part Time MDS Coordinator. This position is part of the on-call rotation with every 5 week obligation and included in the on-call holiday rotation.

The MDS Coordinator oversees and coordinates the development and completion of the resident Assessment ( MDS) in accordance with current Federal and State rules, regulations, and guidelines that govern the resident assessment, including the CAA's.

• Must be a graduate of an accredited School of Nursing, including current licensure by the state of Wisconsin and current AANAC certification preferred but not required.
• Must possess the understanding and ability to comply with Federal and State rules and regulations as it applies to Nursing Homes, such as PDPM.
• Equipment operation: Phone, stethoscope, computer copier, printer, scanner and other nursing equipment.
• Ability to facilitate and communicate with all disciplines.
• Ability to organize and prioritize
• Knowledge of computer applications and processes
