Job summary

Stillwater Post-Acute in El Cajon, CA seeks an MDS Coordinator to lead administrative coordination of the required minimum data set and plan resident care with the inter-disciplinary team.

You will ensure HIPAA compliance, maintain MDS/RAI processes, and communicate with residents, families and regulatory agencies.

RN or LVN license required with 6 months to 1 year of MDS experience; SNF experience preferred; pay $45–$55 per hour; comprehensive benefits offered.

Qualifications

• Education: Graduate of an accredited school of nursing.
• License: Current active license as a Registered Nurse or Licensed Vocational Nurse (LVN).
• Work Experience: At least 6 months to 1 year of MDS experience.
• SNF experience preferred.
• Pay: $45/hr to $55/hr (Depending on experience).

• Provide quality, resident-centered care while protecting resident rights and confidentiality.
• Follow facility policies, HIPAA, and all applicable regulations.
• Maintain knowledge of long‑term care practices, admissions, reimbursement, and MDS/RAI processes.
• Communicate effectively with residents, families, staff, and regulatory agencies.
• Follow safety, infection control, and emergency procedures.
• Prioritize tasks, maintain professionalism, and complete assigned duties.
• Perform other duties as assigned.

Overview of the Position:
