Job summary

Horizons Care Center in Delta, CO seeks a detail-oriented MDS Coordinator to lead the RAI process, ensure timely, accurate assessments, and coordinate care plans.

Requires Colorado RN licensure, 2+ years MDS experience in SNF, RAC-CT and RAC-CTA certifications (or within six to twelve months). Excellent communication and collaboration skills.

Qualifications

• Active Colorado RN licensure.
• Minimum of two years of direct MDS experience in a skilled nursing facility.
• RAC-CT certification or must be obtained within six months of hire.
• RAC-CTA certification or must be obtained within twelve months of hire.

Responsibilities

• Lead and coordinate the RAI process to ensure accurate, timely MDS assessments and care plans.
• Manage RAI process and ensure compliance with regulations.
• Ensure MDS submission accuracy and reimbursement compliance.
• Oversee care planning and coordination, quality, and performance improvement.
• Conduct data analysis and reporting; perform other duties as required.

Salary: $85,000 - $95,000 annually based on experience.

Schedule: Monday - Friday 8am-5pm

What You Bring (Requirements)

• Licensure: Current licensure in the state of Colorado as a Registered Nurse.
• Experience: Minimum of two (2) years of direct MDS experience in a skilled nursing facility (SNF) with solid knowledge of PDMP, case mix systems, and reimbursement methodologies.
• Certifications: Resident Assessment Coordinator - Certified (RAC-CT) certification required or must be obtained within six (6) months of hire.
• Resident Assessment Coordinator - Certified Advanced (RAC-CTA) required or must be obtained within twelve (12) months of hire.
• Strong ability to create, build, and maintain productive relationships at all levels.

• The MDS Coordinator is responsible for leading and coordinating the Resident Assessment Instrument (RAI) process to ensure the accurate, timely, and compliant completion of MDS assessments and care plans.
