The ideal candidate holds a valid NY RN license and has experience in MDS coordination or a related setting, with strong medical terminology knowledge and a commitment to high-quality resident outcomes.

Responsabilités

• Gather information for assessment of residents' physical and mental states.
• Participate in admission process regarding nursing needs and placement.
• Determine potential RUGs and expenses for admissions.
• Review admission applications and communicate assessment results to Admissions Coordinator.
• Complete and assure accuracy of the MDS process for residents.
• Maintain knowledge of Medicare criteria and communicate changes to nursing staff.
• Monitor Case Mix Index (CMI) for Medicaid reimbursement implications.
• Facilitate problem-solving for complex admissions.
• Monitor Medicare assessment schedules and nursing documentation for accuracy and timeliness.
• Assess charts and coordinate with healthcare teams to plan care for current and incoming residents.

Responsibilities:

• Gather information on our skilled nursing facility's current and future patients for future assessment, including physical and mental states.
• Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement
• Determine potential Resource Utilization Groups (RUGs) and expenses associated with a potential admission
• Review all applications for admission, followed by on-site or phone assessment, and communicate results of assessment to the Admissions Coordinator
• Complete and assure the accuracy of the MDS process for all residents
• Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicating changes in regulations
• Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
• Facilitate problem-solving for complicated admissions
• Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission
• Assess charts and communicate with healthcare teams to create applicable health care plans for their current and incoming residents.

Qualifications:

• Valid NY Registered Nurse (RN) license
• Experience in MDS coordinator role or related experience
• Experience in a clinical and healthcare setting
• Deep medical terminology knowledge
