Job summary

Aiken Rehabilitation and Care Center in Aiken, SC, seeks an experienced Registered Nurse Assessment Coordinator (RN MDS Coordinator) to oversee the resident assessment process, ensure timely MDS, CAA and care plan completion, and ensure compliance with state and CMS regulations.

The role requires an active SC RN license and at least one year of MDS/RN assessment coordination experience in a skilled nursing or long-term care setting, with strong organizational and collaborative skills to work

Qualifications

• Current, active RN license in South Carolina.
• Minimum of 1 year of MDS Coordinator experience in a skilled nursing or long-term care setting.
• Thorough understanding of MDS 3.0, RAI process, Medicare, Medicaid and CMS regulations.
• Strong organizational, communication and time-management skills.

Responsibilities

• Complete resident assessments on admission, quarterly, annually and with changes in condition.
• Assess, observe and document changes, communicating findings to the interdisciplinary team.
• Ensure timely completion of MDS assessments, CAAs and care plans.
• Coordinate the interdisciplinary assessment process with nursing, social services and other departments.
• Develop and maintain individualized resident care plans based on assessments.
• Prepare MDS schedules and meet all assessment deadlines.
• Submit MDS transmissions to the state and maintain documentation.
• Monitor assessment accuracy and CMS compliance.
• Assist with regulatory surveys, audits and quality measures.
• Collaborate with facility leadership to promote quality outcomes and compliance.

Registered Nurse Assessment Coordinator (RN MDS Coordinator)

Aiken Rehabilitation and Care Center | Aiken, SC

Aiken Rehabilitation and Care Center is seeking an experienced, detail-oriented Registered Nurse Assessment Coordinator (RN MDS Coordinator) to join our skilled nursing team. In this key role, you will oversee the resident assessment process, ensuring accurate and timely completion of the Minimum Data Set (MDS), Care Area Assessments (CAA), and care planning in compliance with applicable state and federal regulations.

Please note: This is an on-site position and is not eligible for remote work.

Responsibilities

• Complete resident assessments on admission, quarterly, annually, with significant changes in condition, following hospital returns, and as otherwise required
• Accurately assess, observe, and document changes in resident condition while communicating findings to the interdisciplinary team
• Ensure timely completion of MDS assessments, Care Area Assessments (CAA), and comprehensive care plans
• Coordinate the interdisciplinary assessment process with Nursing, Social Services, Activities, Dietary, Rehabilitation, and other departments
• Develop, update, and maintain individualized resident care plans based on assessment findings
• Prepare and maintain MDS schedules to ensure all assessment deadlines are met
• Submit MDS transmissions to the state in accordance with regulatory requirements and maintain accurate documentation and transmission records
• Monitor assessment accuracy and compliance with Medicare, Medicaid, and CMS guidelines
• Assist with regulatory surveys, audits, quality measures, and reimbursement optimization initiatives
• Collaborate with facility leadership to promote quality resident outcomes and regulatory compliance

Qualifications

• Current, active Registered Nurse (RN) license in the State of South Carolina
• Minimum of one year of MDS Coordinator or RN Assessment Coordinator experience in a skilled nursing or long-term care setting
• Thorough understanding of MDS 3.0, the RAI process, Medicare, Medicaid, and CMS regulations
• Strong organizational, communication, and time-management skills
• Ability to work collaboratively with an interdisciplinary team while managing multiple priorities
