Job summary

Park Nursing Home in Rockaway Park, NY is seeking an experienced MDS Assessor to join our in-person team at our Skilled Nursing Facility. The RN-licensed candidate will complete accurate MDS assessments and care plans as assigned.

The role requires long-term care experience, strong organization and communication skills, and the ability to work collaboratively with the nursing staff. MDS experience is preferred, and confidentiality of resident information must be maintained.

Qualifications

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining and updating all care plans and assessments as required.
• Monitoring and auditing clinical records, ensuring accuracy and timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident and Facility information at all times.

Park Nursing Home is hiring an MDS Assessor to work in-person at its Skilled Nursing Facility in Rockaway Park, NY.

We are a 196 bed facility located right by the boardwalk at Rockaway Beach. We have a beautiful view of the Atlantic Ocean and we are accessible to the subway, water ferry and buses.

Required: Registered Nurse (RN) License

DUTIES:

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion

REQUIREMENTS:
