Job summary

Linn County Health Services Public Health Department is recruiting for a Full-Time Nurse Home Visitor for our Babies First and Family Connects programs. This on-site public health nursing position requires two years of community health nursing experience, an Oregon RN license, and the ability to perform home visiting within a defined service area.

The role emphasizes nursing assessments, family education, and collaboration with physicians and community organizations.

Qualifications

• Graduation from an approved school of nursing.
• Two years of current community health nursing experience or equivalent.
• Current license as a Registered Professional Nurse in Oregon.

Responsibilities

• Assess, plan, evaluate and implement community health nursing services in the assigned area or program.
• Conduct home or office visits, provide direct patient care within standing orders, teach prevention and health concepts.
• Coordinate with physicians and health workers to improve patient care; develop community health planning.
• Maintain nursing service records and prepare required reports.
• Foster effective, harmonious work relationships with colleagues and community partners.
