Wexford Health Sources, Inc. is seeking a MOUD/MAT Nurse at Guadalupe Correctional Facility in Santa Rosa, NM.

The role delivers medication-assisted treatment to patients with opioid use disorders, within an interdisciplinary team, and focuses on induction, stabilization, and maintenance phases. As an LPN, you will administer MOUD, monitor responses, educate patients, and ensure regulatory compliance while supporting recovery and continuity of care in a correctional setting.

Qualifications

• Current, unrestricted LPN license in state of practice.
• Current CPR certification.
• Minimum of two years of nursing experience preferred.

Responsibilities

• Administer MOUD medications, including observed dosing, injections, and oral forms.
• Assess and monitor patients through induction, stabilization, and maintenance phases.
• Collaborate with providers, behavioral health staff, pharmacies, and the treatment team.
• Respond to sick calls, provide patient education, monitor medications, and support continuity of care.
• Advocate for patients with a recovery-focused, non-judgmental approach.
• Maintain accurate documentation and ensure compliance with clinical, regulatory, and confidentiality requirements.

Position Summary

As a MOUD/MAT Nurse, you’ll provide skilled nursing care and medication-assisted treatment services to patients with opioid use disorders. Working closely with an interdisciplinary treatment team, you will:

• Administer and monitor MOUD medications, including observed dosing, injections, and oral formulations
• Assess and monitor patients throughout the induction, stabilization, and maintenance phases of treatment
• Collaborate with providers, behavioral health staff, pharmacies, and treatment team members to coordinate patient care
• Respond to sick calls, provide patient education, conduct medication monitoring, and support continuity of care
• Serve as a patient advocate while promoting a non‑judgmental, recovery‑focused approach to treatment
• Maintain accurate documentation and ensure compliance with clinical, regulatory, and confidentiality requirements
