Job summary
AppleGate Recovery in Jackson, TN seeks a Full Time Licensed Counselor for MAT clinics to support adults in Medication Assisted Treatment. You will collaborate with a multidisciplinary team to evaluate progress and develop care plans, conduct assessments, and coordinate services in a compliant outpatient setting.
The role requires TN licensure (LCDAC/CADC/LCSW/CSW/LMFT/LMFTA/LPCC/LPCA) and experience with chemically dependent populations.
Qualifications
- Must hold a Tennessee LCDAC/CADC/LCSW/CSW/LMFT/LMFTA/LPCC/LPCA license.
- Experience with chemically dependent individuals preferred.
- Positive, non-judgmental attitude; ability to collaborate with a counseling team.
Responsibilities
- Provide individual and group counseling; direct services at regular intervals; coordinate services throughout treatment.
- Conduct initial assessment to determine eligibility.
- Complete Bio-psychosocial assessment.
- Establish and review individualized treatment plans.
- Respond to patient grievances and complaints; develop aftercare and discharge plans.
- Provide support for other clinic locations.
- Other duties as assigned by supervisor.
