Welcome to The Jane Pauley Community Health Center! We are seeking qualified therapists who are LCSW, LMFT, LMHC, LCAC. The provider will have the support of in-house wrap-around services including Outreach Specialists, Care Managers, and Patient Care Coordinators.

Job Summary For Licensed Clinical Social Worker (LCSW)

The Licensed Clinical Social Worker (LCSW) is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. The Licensed Clinical Social Worker (LCSW) supports the healthcare team towards a smooth transition from one level of care to another in support of the patient/family.

This position provides the opportunity to serve an established panel of patients in a medically underserved community. The provider will have the support of in-house wrap-around services including Outreach Specialists, Care Managers, and Patient Care Coordinators. JPCHC is a collaborative office known as a pillar of care in Marion County.

Required Education and/or Experience for Licensed Clinical Social Worker (LCSW):

  • Master's Degree in Social Work required.
  • LCSW, LCAC, LMHC, or LMFT licensure required.
  • CPI (Non-Violent Crisis Intervention) certification preferred.
  • Relevant health care, mental health or community-based experience required.
  • Knowledge of the impact of the disease process and human behavior to provide psychosocial assessments, short-term therapeutic counseling, and crisis intervention to patients, families, and staff.
  • Knowledge of state and county adult and child protective legal mandates.
  • Knowledge of principles of growth and development over the lifespan.
  • Principles of crisis intervention and de-escalation techniques.
  • Ability to develop and maintain strong working relationships with the community, medical teams, law enforcement, and protective services.
  • Experience collaborating with physicians, NPs, etc., as part of an interdisciplinary team.
