Summary

The Outreach Social Worker is responsible for participating in the outreach program and providing individual/group counseling and direct service to children, teens, adults and their families. Outreach activities include offsite participation in community events and school outreach to promote mental health awareness amongst the general public. Counseling services range from assessment treatment planning, provision of counseling/psychotherapy and crisis intervention. The Outreach Social Worker is a member of the treatment team and collaborates with other team members.

Essential Functions:

  • Provide brief counseling to at risk students in the school outreach program in multiple public schools.
  • Coordinate and participate in outreach events with other community partners.
  • Maintain and record data entries of outreach encounters to meet scope requirements of funding entities.
  • May perform home and offsite visits as deemed appropriate or necessary by the treatment plan and programmatic needs.
  • Develop group curriculums and execute groups services.
  • Provide direct patient care to clients using a variety of clinical modalities in order to help clients maximize their level of functioning.
  • Maintain appropriate professional patient care documentation in the agency selected Electronic Health Record (EHR), including client assessment and individual notes in compliance with all city, state and federal standards of practice.
  • Adapting to technological information systems to enhance efficiency of documentation and clinical practices.
  • Meet on a weekly basis with supervisor and participate in staff meetings to discuss achievement of program targets, client goals, quality of client care and other reporting requirements.
  • Facilitates collateral collaboration to expand client’s utilization of available support systems and community resources to meet needs identified in clinical evaluation or treatment planning.
  • Conduct care coordination with internal staff or external entities on concerns they may have with clients
  • Meets productivity and data metric requirements, while providing quality of care expectations using clinical interventions and evidence-based models.
  • Performs other related duties as assigned.

  • None

Minimum Qualifications:

  • Master’s Degree in accredited education institute in the disciplines of Social Work, Mental Health Counseling, Art Therapist, or Marriage and Family Therapist
  • Bilingual in one or more of the following: Spanish, Mandarin, Cantonese, Bengali and others, may be required depending on client needs.
  • Proficiency in Microsoft Office.
  • Technologically Proficient.
  • Ability to plan and carry out assignments independently.
  • Ability to prioritize, adhere to timelines and multi-task.

Qualifications Preferred:

  • LCAT, LMFT, LMSW or LCSW certification
  • Proficiency in Electronic Health Records and telehealth platforms.

  • Maintain established departmental policies and procedures including maintain strict confidentiality and adherence to all HIPAA guidelines at all times.
  • Position may require evenings and Saturday hours. The work schedule is subjected to be changed according to the clinic operation needs and is determined based on employee’s ability to meet job requirements and expectations.
