Position Overview

Provides psychosocial services to patients, including in-center and home dialysis patients, using Social Work Theory of Human Behavior and accepted practices. Collaborates with the healthcare team to promote positive adjustment, rehabilitation, and quality of life. Supports quality indicators, outcomes, and process improvements. Key Responsibilities
