Purpose and Scope

Provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients, utilizing Social Work Theory of Human Behavior and accepted social work practices. Collaborates with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for patients. Supports the FMCNA commitment to quality indicators, outcomes, and process improvement activities to help patients achieve quality goals.

Principal Duties and Responsibilities

1. Patient Assessment / Care Planning / Counseling
   • Assess patients' psychosocial status and collaborate on care planning to identify effective interventions.
   • Utilize education programs, social work theories, and quality of life instruments to address barriers and meet treatment goals.
   • Provide counseling and support for patients adjusting to dialysis and seeking transplants.
   • Refer patients to community resources and assist with access to vocational rehabilitation and advance directives.
   • Document interactions and interventions according to policies.
2. Quality
   • Support psychosocial well-being and address adherence and quality of life concerns.
   • Participate in quality review meetings and report on relevant indicators.
3. Patient Education
   • Assess knowledge gaps and provide tailored education about kidney disease and treatment options.
   • Facilitate transplant referrals and educate on psychosocial issues and patient rights.
4. Patient Admission and Continuity of Care
   • Review patient rights and support scheduling and discharge processes.
   • Identify and communicate root causes for discharge requests.
5. Insurance and Financial Assistance
   • Coordinate with insurance and financial departments to address coverage issues and educate patients.
6. Staff Related
   • Assist in staff hiring, training, and team meetings.
7. Other duties as assigned.
