Schedule
Standard operating hours are 8:00 am – 5:00 pm; Monday-Friday. Specific work hours will be established through discussions with your supervisor.

Job Summary
At Buckeye PACE, our Master Social Workers are responsible for direct social work case management services to Mountain View PACE Participants. Provides social services support, including Participant screening, case management, counseling, and referral.
Duties/Responsibilities
- Participate in the Interdisciplinary Team’s initial assessments, care planning, and ongoing re-evaluations of the Participant’s case.
- Maintain current written case management records, including periodic reassessment of program Participants.
- Provide individual and family counseling; develop and lead group counseling and activities.
- Refer Participants to appropriate community agencies or facilities, act as a liaison with such organizations, and advocate for Participants.
- Create effective relationships among staff to build and maintain a cohesive team.
- Consult with and advise staff on social, emotional, and cultural factors related to health and medical care, and the availability of social services in the community.
- Participate in program and policy development of the social work component.
- Communicate Participant changes to team members.
- Participate in ongoing Quality Initiative training as directed.
- Other duties as assigned.

- CPR and First Aid Certification.
Education and Experience
- Master’s Degree in Social Work (MSW) from an accredited school with one year of experience working with the frail/elderly population.
- One year of experience working with the frail and elderly population or willingness to participate in extensive training.
- Valid state driver’s license and reliable transportation required.
Requirements
- Valid State driver’s license with a clean driving record.
- Successful completion of background check, physical, drug screening, TB screening.
Physical Requirements
- Work inside the facility in various areas.
- Sit, stand, bend, lift, and move intermittently during work hours.
- Frequent interruptions are expected.
- Maintain alertness for Participant’s safety.
- Lift, carry, and support Participants as needed.
