Job summary

Aleca Hospice is seeking a Master Social Worker to implement standards of care for medical social work services and ensure adherence to service agreements.

The role involves psychosocial assessments, crisis intervention, family education, and collaboration with an interdisciplinary team to support patients with terminal illnesses. A master’s or baccalaureate in social work and CPR certification are required.

Qualifications

• MSW or BSW with CSWE accreditation required.
• Minimum 1 year healthcare/hospice experience.
• CPR certification required.

Responsibilities

• Assess psychosocial status of patients and families in terminal illness context.
• Conduct social evaluations, address crisis intervention, and support coping.
• Educate team on cultural needs and family dynamics.
• Maintain clinical records for all referred patients.
• Provide information and referrals for practical/environmental needs.
• Participate in development of individualized care plans with family input.
• Assist physician and team in understanding psychosocial factors.
