myPlace Health in Los Angeles, CA is seeking a Master’s level Social Worker to join their PACE program on-site. You will plan, organize, and deliver social work services to support participants and families with dignity in the community.

You will maintain a caseload, collaborate with the Interdisciplinary Team, assist with Medi-Cal applications, and coordinate transitions to promote independence at home. This role emphasizes compassionate, person‑centered care.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited university.
• CPR and First Aid certification required (or must be obtained within 30 days of hire).
• 2 years of experience conducting psychosocial assessments, care planning, and case management.
• 2 years of experience working with frail or elderly populations.
• Current California Driver's License with a clean driving record and personal automobile insurance.
• Proficiency in EMR/EHR documentation preferred.
• Experience collaborating with multidisciplinary or medical teams preferred.
• Bilingual English/Spanish, Cantonese, Mandarin, or Vietnamese is a plus.
• Ability to lift and carry up to 25 pounds.
• Onsite schedule: Monday–Friday 8am–5pm at our Los Angeles center.

Responsibilities

• Engage in daily IDT meetings and conduct psychosocial assessments with care planning.
• Present assessments at care plan conferences for IDT collaboration.
• Guide participants through transitions between hospitals, home, and care facilities.
• Connect participants, families, and caregivers with community resources and support.
• Support Medi-Cal applications and coordinate with government agencies as needed.
• Coordinate advanced care planning with providers to address goals of care.
• Design and lead group counseling sessions to promote well‑being and engagement.
• Document care plans and interactions accurately in the EMR.
• Provide discharge planning support and assist with next steps after program ends.
• Embrace teamwork and be flexible to take on additional duties.
