Job summary

myPlace Health in Los Angeles, CA is seeking a licensed MSW to join our PACE center onsite. You will plan, organize, and deliver social work services tailored to each participant, maintaining a caseload and collaborating with the Interdisciplinary Team (IDT) to ensure person-centered care.

You will engage in IDT meetings, present assessments, coordinate transitions, connect families with resources, and support Medi-Cal processes while ensuring care coordination across settings.

Qualifications

• MSW from an accredited university.
• CPR and First Aid certification required.
• 2 years of psychosocial assessments, care planning, and case management.
• 2 years of experience with frail or elderly populations.
• Current California Driver's License with clean driving record.
• Proficiency in EMR/EHR documentation; experience with multidisciplinary teams.
• Bilingual abilities in English and Spanish, Cantonese, Mandarin, or Vietnamese are a plus.
• Ability to lift up to 25 pounds.

Responsibilities

• Engage in daily IDT meetings and develop individualized care plans.
• Present assessments at care plan conferences with the IDT.
• Coordinate smooth transitions between hospitals, homes, and facilities.
• Connect participants, families, and caregivers with community resources.
• Assist with Medi‑Cal applications and coordinate communications with government agencies.
• Design and lead group counseling sessions.
• Document care plans and interactions accurately in the EMR.
• Provide discharge planning support when transitioning out.
• Support flexibility and other duties as assigned.

As an MSW, you’ll plan, organize, and deliver social work services tailored to the unique needs of each individual.In this role, you’ll maintain a caseload of participants, collaborating closely with the Interdisciplinary Team (IDT) to ensure seamless, person-centered care. Acting as a trusted liaison between the PACE program and participants, families, and caregivers, you’ll foster meaningful relationships that support seniors in continuing to live independently at home.
