Southcoast Behavioral Health, located in Dartmouth, Massachusetts, is a premier mental health treatment center with 192 beds. We serve pediatric, adolescent, adult, and geriatric patients, as well as adults with co-occurring mental health and substance use disorders.

Purpose Statement

Manage the client's/patient's/resident’s cases with a solid background of clinical knowledge and expertise. Responsible for providing therapeutic services to clients/patients/residents.

Caseload of 12.

Work closely with a team including another Case Manager, attending providers, nurses, and BHA’s to engage patients and develop safe discharge plans.

Complete treatment plans, psychosocial assessments, and hold bi-weekly meetings with each patient.

Collaborate with families and community providers, including DMH, to coordinate care and discharge plans.

Work Schedule: Monday-Friday, 8am-4pm.

Responsibilities

1. Provide case management and care coordination services.
2. Coordinate continuum of care activities, ensuring resource utilization and compliance.
3. Conduct psychosocial assessments and facilitate individual, family, and group sessions.
4. Coordinate treatment and discharge activities.
5. Develop, implement, manage, and evaluate care plans.
6. Collect and analyze clinical data for the treatment team.
7. Communicate medical and clinical information with insurance providers and the treatment team.
8. Coordinate issues relevant to treatment team in a timely manner.

Qualifications

• Bachelor’s degree in Social Work, Counseling, or related human service field.
• One or more years’ experience in a behavioral health setting preferred.
• Experience with the patient population served is preferred.
• Experience in case management and utilization review is preferred.

Licenses/Certifications

• Current license as required by the state.
• CPR and de-escalation/restraint certification (training provided upon hire).
• First aid may be required depending on state or facility regulations.
