Stonebridge Home Care in Utah is seeking a Marketer to manage referrals and patient intakes. This full-time Community Liaison role emphasizes building strong relationships with hospitals, SNFs, case managers, and other partners.

It offers growth toward a Director of Marketing as the agency expands and services increase. Ideal candidates will have excellent communication, organizational skills, and the ability to work independently while managing multiple priorities in a dynamic home health

Responsibilities

• Manage incoming referrals from hospitals, SNFs, case managers, and other partners.
• Generate new referral sources.
• Perform patient intakes and execute service agreements.
• Coordinate care initiation promptly to reduce delays and improve patient experience.
• Maintain accurate referral and intake trackers.
• Build and maintain strong relationships with referral sources through regular communication and check-ins.
• Generate new referrals from established referral sources.
• Identify opportunities for growth and assist with marketing initiatives as needed.
• Serve as a liaison between referral sources, patients, and internal care teams.

Position Summary

We are seeking a Marketer to join our team and play a dual role in referral source management and patient intake. This position relies on relationship-building skills to ensure we maintain close relationships with referral sources and build strong relationships with new referral sources. Full-time Community Liaison role with opportunity to grow into a Director of Marketing role as Stonebridge continues to grow.

Key Responsibilities

• Manage incoming referrals from hospitals, skilled nursing facilities (SNFs), case managers, and other partners.
• Generate new referral sources.
• Perform patient intakes and execute service agreement contracts as needed.
• Coordinate care initiation promptly to reduce delays and improve patient experience.
• Maintain accurate referral and intake trackers.
• Build and maintain strong relationships with referral sources through regular communication and check-ins.
