Job description

Position: Market Physician Executive (MPE) - Alabama

Monogram Market Physician Executives (MPEs) are mission-driven physician leaders dedicated to improving patient well-being, quality of life, and health outcomes. They lead our in-home multi-specialty care model in their assigned markets, which include 5-10 practices led by advanced practice providers, nurses, social workers, and pharmacists. MPEs collaborate with Monogram’s Multi-Specialty Platform to leverage specialists for in-home care, utilizing AI algorithms to predict appropriate care levels, aligning with the quadruple AIM to enhance patient experience, population health, provider satisfaction, and cost reduction. The primary goal is to achieve exceptional outcomes through disease detection, evidence-based pathways, and treatment.

Reporting to the Region President, the MPE is a key clinical leader responsible for developing and overseeing clinical strategies, policies, and operations that improve patient outcomes. They oversee daily clinical and business operations, including direct patient care, care management, and support services, and review and approve care plans for advanced practice providers. Collaboration with community physicians and facilities is essential for peer-to-peer and direct care decision-making.

Roles and Responsibilities
Clinical Guidelines Execution – 25%
  • Understand and implement Monogram’s proprietary clinical pathways.
  • Ensure adherence to clinical guidelines and the Model of Care.
  • Review and approve care plans of APPs, RNs, SWs, and PharmDs.
  • Document clinical activities accurately within Salesforce and Athena, and co-sign encounters.
Clinical and Operations Performance & Quality Improvement – 25%
  • Reduce total cost of care and Medical Loss Ratio.
  • Manage clinical outcomes, including interventions, utilization, pharmacological therapy, and quality measures.
  • Lead daily review rounds for high-risk cases.
  • Supervise clinical and operational staff.
  • Oversee daily operations through the Market Manager, including patient engagement, scheduling, and P&L management.
  • Present performance metrics and outcomes to leadership.
Patient Care and Treatment – 25%
  • Provide direct and indirect patient care, including diagnosis and treatment.
  • Engage with patients on treatment plans and collaborate with community providers.
  • Conduct peer-to-peer reviews with community, facility, and health plan partners.
  • Order labs, referrals, and execute actions to improve outcomes and close care gaps.
Population Health Management (PHM) – 20%
  • Guide population health activities, including high-risk patient interventions and readmission reduction strategies.
  • Collaborate with finance and other stakeholders on utilization trends.
  • Present and lead population health strategies in meetings.
  • Review patient cohorts to identify high utilizers and opportunities for intervention.
Miscellaneous – <5%
  • Participate in on-call activities (minimum once/quarter).
  • Provide coverage for colleagues during PTO or vacancies as needed.
Position Requirements
  • Remote position within the market, with occasional in-home visits and travel.
  • Experience applying evidence-based clinical criteria.
  • Experience in renal care and geriatrics preferred.
  • Strong management and communication skills.
  • Active, unrestricted medical license in the state(s) within the market.
  • Experience with Medicare Advantage, Medicaid, NCQA, HEDIS, and risk adjustment.
  • MD or DO degree; BC or BE in a relevant specialty (e.g., Nephrology, Internal Medicine, Geriatrics).
  • Willingness to obtain multi-state licensure.
