Position: Market Physician Executive (MPE) - Charleston, SC

Monogram Market Physician Executives (MPE) are mission-driven physician leaders dedicated to improving patient well-being, quality of life, and health outcomes. The MPE will lead our in-home multi-specialty care model in the market, collaborating with local practices and specialists to deliver in-home care, leveraging AI algorithms to predict care needs, and focusing on the quadruple AIM: patient experience, population health, provider satisfaction, and cost reduction.

Reporting to the Region President, the MPE oversees clinical strategies, policies, and daily operations, including direct patient care, care management, and social and pharmacy services. The MPE reviews and approves care plans, collaborates with community physicians, and participates in peer-to-peer and patient care decision-making.

Roles And Responsibilities
Clinical Guidelines Execution – 25%
  • Implement and ensure adherence to proprietary clinical pathways and guidelines.
  • Review and approve care plans from APPs, RNs, SWs, and PharmDs.
  • Document clinical activities within Salesforce and Athena, co-sign APP encounters.
Clinical and Operations Performance – 25%
  • Reduce total cost of care and improve clinical outcomes.
  • Lead high-risk reviews, supervise clinical and operational teams, and oversee daily operations and P&L through the Market Manager.
  • Present market performance to leadership.
Patient Care and Treatment – 25%
  • Provide direct and indirect patient care, engage with patients on treatment plans, and collaborate with community providers.
  • Order labs, referrals, and complete actions to improve outcomes and close care gaps.
Population Health Management – 20%
  • Guide population health activities, identify high-risk patients, and develop strategies to reduce hospital admissions.
  • Collaborate with stakeholders to analyze utilization trends and improve care quality.
  • Lead patient reviews targeting high utilizers and high-risk groups.
Miscellaneous
  • Participate in on-call activities (minimum once per quarter).
  • Provide coverage for other MPEs as needed.
Position Requirements
  • Remote within the market with occasional in-home visits and travel.
  • Experience applying evidence-based clinical criteria, especially in renal care and geriatrics.
  • Strong management and communication skills.
  • Active, unrestricted medical license in the state(s) within the market.
  • Experience with Medicare Advantage, Medicaid, NCQA, HEDIS, and risk adjustment.
  • MD or DO degree from an accredited institution, BC or BE in relevant specialties.
